Aussie Pet Mobile of Walnut Creek Pet Release Form Aussie Pet Mobile requires that each pet being groomed is current on its rabies vaccination. While Aussie Pet Mobile adheres to stringent sanitation procedures for our mobile vans, we advise that each pet we care for be vaccinated against distemper, hepatitis, par influenza adenovirus type 2, parvovirus, corona, leptospirosis and bordetella prior to being groomed. Aussie Pet Mobile requires that you disclose any health issues, conditions or diseases that your pet may have prior to grooming. As well as any Covid-19 issues in the household. The health and safety of your pet is our first priority. Prior to or during the grooming process, the groomer may determine that the treatment may place the health or safety of the pet or the groomer at risk. This may be due to the age, health, stamina or disposition of the pet. The groomer may also decline to perform an extensive de matting of a dog or cat in order to prevent inhumane discomfort and/or pain to the pet. A shave down (instead of de matting) may be the most appropriate procedure for severely matted pets. Should such a situation arise, the groomer will halt the grooming process and inform the pet owner of the situation. Aussie Pet Mobile reserves the right to refuse grooming any animal that may be a threat to itself or any person. Inability to take a cat from its carrier or muzzle a dog may be grounds for refusing service. Aussie Pet Mobile does not use any type of tranquilizers on any pet. Drying a pet may be hindered by its inability to remain standing during the drying process or its adverse reaction to our drying equipment. The groomer will inform the pet owner of such a situation and make appropriate suggestions to remedy the issue. We use natural and chemical solutions to treat pets for fleas and ticks. Some animals are sensitive to these solutions and may experience an allergic or other reaction. If we notice any problems with the animal, you will be notified immediately. While Aussie Pet Mobile’s grooming procedures are designed to avoid hurting or injuring your pet, some steps in the process (particularly shave downs) may result in cuts and/or abrasions to the skin. Shaving a pet often exposes pre-existing skin conditions. If an animal is badly matted, you may find sores and tender skin under those mats. Sometimes the skin condition is such that the clippers may cut or irritate the skin. Especially while shaving severely matted pets. While the groomer will employ the greatest care to avoid such events, they do occur in the normal practice of grooming pets. For the unforeseeable future, Aussie Pet Mobile has instituted a non-contact payment method. Aussie Pet Mobile charges a $25.00 fee for returned checks, in addition to any fees levied by any financial institution. AUTHORIZATION: I am the owner/caregiver of this/these pet(s), and I have read and understand the above cautions/requirements. I acknowledge the risks inherent to pet grooming, as described above, that may cause injury, allergic and/or other to reactions to my pet(s). I desire to have Aussie Pet Mobile perform the grooming. I consent to and authorize the grooming of my pet(s).
